We invite attorneys to contact our office for a complimentary consultation with Harold Bialsky to review case details. Upon engagement, you will receive an initial email containing: 

• A retainer invoice representing 50% of the total fee for a Life Care Plan and/or
  Vocational Assessment. This payment is required to initiate services. The final report will
  be released upon receipt of the balance invoice.
• A Dropbox link to upload required documentation (see below).
• A scheduled date and time for the client’s Zoom interview with Harold Bialsky

An expedited service fee applies to reports requested within four weeks or less.

1. Appropriate legal documentation.
2. Medical records: hospital admission/discharge summaries, operative reports, IME/narrative reports, treating physician reports
3. Deposition/EBT transcripts of plaintiff

4. Tax returns and W-2s (pre-incident to present)
5. If available, employment records and where applicable, educational records.
